Medication reviews are normally arranged when due.

Your request will be reviewed by our Pharmacy Team before an appointment is booked.

Submitting this form does not guarantee that a medication review appointment will be required.

Please specify which day is best for our Pharmacist to call you and whether your preference is morning or afternoon.

Please be aware that we cannot provide an exact call time.

If you have online access, you may see an appointment time once booked. Please ignore this, as the pharmacist may call at any point during your selected morning or afternoon session.

What adjustment do you believe would help us complete your medication review?
