Registering as a New Patient
We welcome new patients to Horizon Healthcare.
You can register with the practice online or by contacting our reception team for support.
How to Register
To register with us, please complete our online registration form.
You may be asked to provide:
• Your NHS number (if known)
• Proof of identity
• Details of any current medication or medical conditions
Our team will process your registration and contact you if any further information is required.
What happens after you register
Once your registration has been completed:
• You will be able to access our services
• You may be invited for a new patient health check
• Our team may contact you if further information is needed
If you take regular medication or have ongoing health conditions, you may need to book an appointment with a clinician before prescriptions can be issued.
How care works at Horizon Healthcare
When you contact the practice, your request will be reviewed to ensure you are directed to the most appropriate clinician or service.
This means you may be offered an appointment with a:
• GP
• Nurse
• Pharmacist
• Physiotherapist
• Mental health practitioner
• Other appropriately trained clinician
In some cases, you may also be directed to a more suitable service such as a pharmacy or urgent care service.
This approach helps ensure you receive the right care as quickly as possible.

If you are unable to use online services, our reception team is available to help.
Patients Living Outside Our Area
If you live outside our practice boundary, you can still request to register with us. These registrations are known as Out-of-Area (OOA) registrations and are considered on an individual basis.
While we are happy to provide your care at the surgery, there are some important limitations.
Important Information for Out-of-Area Patients
If you live outside our local area:
• We are unable to provide home visits or urgent care at your home
• Some referrals may need to be arranged through services closer to where you live
• We may not automatically receive test results from hospitals outside our area
• Some services (such as community teams, screening, and vaccinations) are organised based on where you live
What this means for your care
You will still be able to access appointments and care at the practice.
However, depending on your needs, you may be:
• Directed to services closer to your home
• Asked to share results or information from other providers
• Advised to contact local services for certain types of care
Choosing the right practice
In some cases, it may be more appropriate to register with a GP practice closer to your home. This can help ensure you have full access to local services, including home visits and community care.
