Horizon Healthcare is an NHS GP practice providing care across our Burton Road and Mackworth sites.

We offer multiple ways to access healthcare, including telephone support, online services, face-to-face support, and 24/7 automated options for many routine requests.

Whether you prefer digital services or speaking to someone directly, our aim is to make accessing care as simple and convenient as possible.

Same-Day Urgent Care Where Appropriate

Our care navigation and clinical teams work together to ensure patients with urgent medical problems are reviewed appropriately.

Depending on your needs, you may be supported by:

  • GPs
  • Nurses
  • Pharmacists
  • Physiotherapists
  • Mental Health Practitioners
  • Advanced Clinical Practitioners
  • Other appropriately trained clinicians

In some cases, you may also be directed to another suitable NHS service, such as a pharmacy or urgent care service.

This helps ensure patients receive the right care from the right professional as quickly as possible.

Many urgent problems can be assessed on the same day where clinically appropriate.

We understand that not everyone wishes to use online services.

Patients can still contact us by telephone or speak to our reception team for assistance.


How to Register

The quickest and easiest way to register is online.

You do not usually need proof of address, immigration status, ID, or an NHS number to register with a GP practice.

If you need help registering, our reception team will be happy to assist.


What Happens After You Register?

Once your registration has been processed:

  • Your medical records will be requested from your previous GP practice
  • You will be able to access our services
  • You may be invited for a new patient health check
  • Our team may contact you if further information is required

Please allow several working days for registrations to be processed.


Important Information About Medication

If you take regular medication or have ongoing medical conditions, you may need an appointment or medication review before some prescriptions can be issued safely.

This helps ensure your medication and monitoring remain safe and up to date.


Need an Appointment?

We offer a range of ways to contact the practice depending on your needs and preferences, including online services, telephone support, and face-to-face assistance.

If you are unable to use online services, our reception team will be happy to help.


Patients Living Outside Our Area

Patients living outside our practice boundary can still request to register with us. These are known as Out-of-Area (OOA) registrations and are considered individually.

Important Information for Out-of-Area Patients

If you live outside our local area:

  • We may not be able to provide home visits
  • Some services may need to be arranged closer to where you live
  • Some community services are organised based on your home address

You will still be able to access appointments and care at the practice.

In some cases, registering with a GP practice closer to your home may provide easier access to local community services and urgent home support.

If you are unsure, our team will be happy to discuss your options.


Help With Online Services

The NHS App allows patients to:

  • Order repeat prescriptions
  • View parts of their medical record
  • Receive messages from the practice
  • View appointments
