Select a number from 0 to 5, where 0 means you never cough and 5 means you cough all the time
Select a number from 0 to 5, where 0 means you have no phlegm (mucus) in your chest and 5 means your chest feels full of phlegm
Select a number from 0 to 5, where 0 means your chest does not feel tight at all and 5 means your chest feels very tight

An exacerbation is where your breathlessness or cough got worse and you needed to take a rescue pack or to seek medical attention
