If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).

To update your name on your medical record, we require evidence of your name change.

Accepted documents include:
• Marriage Certificate
• Civil Partnership Certificate
• Deed Poll
• Decree Absolute
• Other official legal documentation confirming your name change

Change of Address

Please ensure you include all address details, including any flat number, apartment number, room number, building name and postcode where applicable.

Adult patients (aged 18 and over) must submit their own change of address request.
