About your request

If you feel well enough to return to work before your fit note expires, you can usually return to work without obtaining a new certificate from your GP.

Horizon Healthcare does not provide:

  • Return to work certificates
  • Fit for work certificates
  • Occupational Health assessments

Please discuss your return to work with your employer.

What do you need?

You do not require a fit note for the first 7 calendar days of sickness.

Most employers will accept self-certification during this period.

A fit note may require a clinical assessment before it can be issued.

If your condition has not been assessed by an NHS healthcare professional, you may need an appointment before a fit note can be provided.

Are you currently under the care of a hospital specialist for this condition?
Has the hospital team advised whether they can provide a fit note?

If the expiry date is more than 2 working days away fit notes cannot be issued in advance.

Clinicians are able to backdate fit notes where clinically appropriate but cannot issue future-dated certificates.

Please submit your request again within 2 days of your current fit note expiring.
