Register for Online Services

Use this form if you:

  • Want to register for online access to your own medical record
  • Want to link your medical record to the NHS App or another approved application

This form must only be completed by the patient themselves.

If you require access to another person’s medical record, please use the Request Proxy Access to Online GP Services form instead.

If you wish to give another person permission to discuss your medical information, please use the Consent to Discuss Medical Information form instead.

Identity Verification

For first-time registration requests and password resets, we will require photographic identification before access can be granted.

Accepted forms of identification include:

  • Passport
  • Driving Licence
  • Other government-issued photographic identification

Please upload identification or your request will be closed without action.

To complete your registration please upload proof of identity, this should include Photographic ID and proof of address.

Terms and Conditions

  • I understand that I can terminate my account at any time by contacting the surgery, or change my log in details by re-registering and that this form will be kept on my electronic records.
