Use this service to request proxy access to another person’s online GP services, including access through the NHS App or linked profiles.
Proxy access may allow another person to:
- order repeat prescriptions
- book appointments
- view elements of the medical record
All requests are reviewed individually and access may be limited, restricted, or refused where appropriate.
You can only use this service if the patient is registered at the surgery.
Before you start
We will ask you for:
- patient details
- requester details
- relationship to the patient
- confirmation of consent where appropriate
