If you have been asked to arrange a blood test, please follow the steps below.

Call 01332 502735
Select Option 1, then Option 1 again
Enter the PIN provided in your message
Follow the instructions to confirm your appointment
📄 Blood test forms
If your appointment is at the surgery, your form will be printed for you.
If your test is elsewhere, you will need a form.
You can:
- Collect it from reception, or
- Request it online using our Repeat blood and urine test form
Your form can then be sent to you by SMS or email.

- Appointments are available up to 14 days in advance
- If no appointments are available, please try again later
- New appointments are released regularly
