Some conditions and medications require regular monitoring to ensure your treatment remains safe and effective.
If you have been asked to arrange monitoring, please follow the steps below.

Appointments are 20 minutes
🩺 What happens at your appointment
The nurse will complete all required monitoring during one appointment.
This may include:
- Blood pressure check
- Pulse check
- Weight measurement
- Blood tests (if required)
- Urine sample (if needed)
- Other checks related to your condition
If blood tests are needed, these are usually taken during the same appointment.
📄 Blood tests elsewhere
If you prefer to have your blood tests done elsewhere, these can be booked via SwiftQueue.
You will need a copy of your blood test form.
You can:
- Collect it from reception, or
- Request it online using our Repeat blood and urine test form
Your form can then be sent to you by SMS.
You will still need to book a nurse appointment with the practice for your physical checks (e.g. blood pressure, foot check).

- Appointments are available up to 14 days in advance
- If no appointments are available, please try again later
- New appointments are released regularly
