Asthma control

Past 4 weeks
During the past 4 weeks, how often did your asthma prevent you from getting as much done at work, school or home?
During the past 4 weeks, how often have you had shortness of breath?
During the past 4 weeks, how often did your asthma symptoms (wheezing, coughing, shortness of breath, chest tightness or pain) wake you up at night or earlier than usual in the morning?
During the past 4 weeks, how often have you used your reliever inhaler (usually the blue inhaler) or nebuliser medication?
How would you rate your asthma control during the past 4 weeks?

Exacerbations

Past 12 months
An exacerbation is when symptoms worsen and medical help is needed
Has the child needed oral steroids for asthma in the past 12 months?
Has the child attended A&E or been admitted to hospital for asthma in the past 12 months?

Medication and adherence

Is the child prescribed a preventer inhaler (usually brown/orange/purple)?
How often is it taken?
Are there any problems with the child’s asthma medication?
For example, side effects, concerns, not helping

Inhaler technique and equipment

Does the child use a spacer with their inhaler (if prescribed)?
Do you feel confident the inhaler is being used correctly?

Environment (smoking exposure)

Does anyone in the household smoke or vape?
Is the child regularly exposed to smoke?
