Do you need a sick note?
If you have been off work for 7 days or less, you do not need a GP sick note.
You can complete a self-certification form with your employer.
Some employers may still request a doctor’s note during this period. This may involve a charge.
If you have been off sick for more than 7 days
You may need a sick note from a clinician.
We can:
- Issue new sick notes for conditions that have already been assessed
- Provide extensions without an appointment where appropriate
If you have not been assessed for your condition, you will need to contact the practice to arrange an appointment.

Your request will be sent directly to the team and is usually processed within 3 working days.

You can also request a sick note using our online form.
Please allow up to 5 working days for processing.
Returning to work
You do not need to see a GP to return to work before your fit note ends.
A fit note is advice only, and you can return to work earlier if you feel well enough.
If your employer has concerns, they should carry out a workplace risk assessment or seek occupational health advice.
