Sick (Fit) Notes

A fit note (sometimes called a sick note or Statement of Fitness for Work) may be required if you have been unable to work due to illness.

The quickest way to request a fit note is through our automated telephone service, available 24 hours a day, 365 days a year.

When Do I Need a Fit Note?

You do not normally need a fit note for the first 7 calendar days of sickness.

For the first 7 days, you can usually self-certify your sickness directly with your employer.

You may need a fit note if:

  • You have been absent from work for more than 7 calendar days
  • You require an extension to an existing fit note
  • A healthcare professional has advised that a fit note is appropriate

Hospital-Issued Fit Notes

If you have recently been treated by a hospital consultant, attended an outpatient appointment, undergone surgery or been discharged from hospital, the hospital team should normally provide any fit note relating to that episode of care.

Please check with the hospital department before requesting a fit note from your GP practice.


Returning to Work

If you feel well enough to return to work before your fit note expires, you can usually return to work without obtaining a new certificate from your GP.

A fit note is advice and does not need to expire before you can return to work.

Please discuss your return with your employer.


Fit for Work Certificates

Horizon Healthcare does not provide:

  • Fit for work certificates
  • Return to work certificates
  • Fitness to resume employment certificates
  • Occupational Health assessments

If your employer requires an assessment of your ability to undertake specific duties, they should arrange this through their Occupational Health service.


Before You Request a Fit Note

We will ask you:

  • Whether this is a new fit note or an extension to an existing fit note
  • How long you have been off work
  • Details of your illness or condition
  • Whether you have already been assessed for the condition
  • Whether you are currently under the care of a hospital specialist

If your condition has not previously been assessed, you may need an appointment before a fit note can be issued.
