Before booking your medication review appointment, you must complete all required monitoring checks.

- Any blood tests requested
- Any physical check requested – eg blood pressure, footchecks, ECG etc
These checks must be completed before booking your medication review.
This ensures your clinician has up-to-date information to safely assess and prescribe your medication.
How to Book Your Medication Review
Once all monitoring has been completed:

Timeframe
All required monitoring must be completed within 8 weeks of receiving this message.
If monitoring is not completed within this timeframe, your medication may be reviewed and adjusted for safety reasons.
This is to ensure your treatment remains safe and clinically appropriate.
