They are often based in the community rather than at the surgery, and you may be referred to them by a member of our team based on your needs.
They play an important role in supporting patients with ongoing, complex, or specialist needs.
Some of these services are provided in partnership with our Primary Care Network (PCN), working across local practices to support patient care.
🏥🏠 District Nursing Team
The District Nursing Team provides care for patients who are unable to attend the surgery, including care delivered at home.
They can help with:
• Wound care and dressings
• Ongoing health needs
• Support for housebound patients

🤰 Community Midwives
The Community Midwifery Team is based at London Road Community Hospital and provides care and support during pregnancy and after birth.
If you are pregnant, please contact the surgery on 01332 737777. We will arrange your first appointment, which usually takes place when you are between 8–10 weeks pregnant.

👶 Health Visitors
Health visitors are specialist nurses who focus on health promotion and the prevention of illness.
They offer advice and support on a range of issues affecting the health and wellbeing of children and families.

Care Coordinators support patients with additional needs, helping to organise care and connect with appropriate services.
They can help with:
• Coordinating care plans
• Supporting care home patients

Social Prescribers support patients with non-medical needs that may be affecting their health and wellbeing.
