Urine Sample (ACR Test)

You have been asked to provide a urine sample to check your kidney health.


🧪 What you need to do

  • Collect a first morning urine sample where possible
  • Bring the sample on the same day

📄 Test request form

You will need a paper request form for your sample.

You can:

If you are attending a nurse appointment, the form can be printed for you at the appointment.


📍 Sample pots

Sample pots can be collected from the surgery.


📦 Where to return your sample

You can return your sample:

  • To the surgery, or
  • To the hospital (if attending an appointment)

At the surgery:
Please place your sample in the specimen box on the wall in Building 1.
Do not hand samples to reception.
Ensure your sample is bagged with the request form.


🏷️ Labelling your sample

Your sample must include:

  • Full name
  • Date of birth
  • Date of sample
  • Time of sample

Samples without this information may be rejected by the lab.


🩺 Why this is important

This test helps us check how well your kidneys are working.

Some medications require regular monitoring to ensure they remain safe.

If results are not available, your clinician may need to review, reduce, or stop certain medications until it is safe to continue.
