Living with a disability can affect many areas of life, and a range of local and national services are available to help.


Get help from our team

Our Community Support Team (Social Prescribers and Care Coordinators) can support you with:

  • Accessing local disability support services
  • Help with benefits, housing or daily living
  • Connecting you with community groups and support networks
  • Accessing equipment or practical support

Accessibility and reasonable adjustments

If you have a disability or need support accessing our services, we can provide reasonable adjustments.

This may include:

  • Interpreters or communication support
  • Accessible information (large print, easy read)
  • Longer appointments where needed
  • Help using online services
