Accessing your health record

Your health record includes information such as your conditions, allergies, medications, test results, and clinical notes.


View your record online

You can access your health record using:

Most patients will automatically have access to additional information in their record, including letters, test results, and appointment notes.


Access to your full health record

Some parts of your record may require additional access.

If you do not have full access, you may only be able to see your medications and allergies.

To request access to your full health record:

Once approved, you will be able to view your full record online

Complete a registration form and request full record access

Provide photo ID (online or at the surgery)


Access for someone else

If you would like to access someone else’s health record (for example as a parent, carer, or with consent), you will need to apply separately.

Accessing other records

We can only provide access to information that has been added to your record by our practice.

We are the data controller for the information we record.

If information has been added by another service (such as a hospital), we may not be able to share this directly.

To request records from another service, please contact them directly.
