You may be able to access someone else’s health record if you are their parent, family member, or carer. This is called proxy access.
Proxy access can be set up if you are both registered with the practice.
Request proxy access
To request access, please complete our online form:
👉 Request access to someone else’s record
We will review your request and may contact you for identification or further information.
Using proxy access
Once access has been set up, you will be able to view the other person’s record using your NHS account (NHS App or website).
You can switch between profiles using linked profiles.
Children aged 13 to 16
When a child reaches the age of 13, proxy access is automatically removed.
At this age, young people have the right to make decisions about who can access their medical information. This is to protect their confidentiality.
In most cases, ongoing proxy access is limited. Where access is appropriate, this is usually restricted (for example, to allow repeat medication requests only).
Full access to the medical record is not routinely provided and will only be considered where there is a clear clinical need and it is in the patient’s best interests.
Parental involvement in a young person’s care can still continue, and consent to discuss specific issues can be agreed separately where appropriate.
