Blood pressure review

Use this form to submit home blood pressure readings requested by the practice.

You can use this form if you:

• are registered at Horizon Healthcare
• have been asked to submit blood pressure readings by the practice

Before you start

We will ask you for:

• your name, date of birth, postcode, telephone number, and email address
• details of the person you are completing the form for (if applicable)
• up to 7 days of blood pressure readings

Taking blood pressure readings at home

Please take your blood pressure when you are relaxed and seated.

If possible:

• take a reading in the morning and in the evening
• continue for up to 7 days

Please submit all readings requested by the practice.

Important

Do not use this form for urgent medical problems.

If you have:

• chest pain
• severe shortness of breath
• symptoms of a stroke
• very high blood pressure with symptoms such as severe headache, blurred vision, or confusion

please seek urgent medical help by calling 999 or NHS 111.

What happens next

Your readings will be reviewed by the appropriate team.

If further review, medication changes, or appointments are needed, we will contact you.
