Use this service to request access to Horizon Healthcare’s online services.
Online services allow you to access a range of healthcare services without needing to contact the practice.
Depending on the access granted, you may be able to:
- Order repeat prescriptions
- View your medication
- View test results
- View appointments
- Access parts of your medical record
- Use NHS App services
Before You Apply
To protect your personal information, we need to verify your identity before online access can be granted.
You will be asked to upload:
- One form of photographic identification (for example, a passport or driving licence)
- Proof of address (for example, a utility bill, bank statement or council tax letter dated within the last 3 months)
Applications cannot normally be processed without suitable identification.
Important Information
Access to your full medical record is not provided automatically.
If you later request access to historical medical records, this may require a review of your medical record before access can be granted.

- Proof of address
