Hi again!

Welcome back to the Horizon Healthcare patient newsletter. Here we will keep you updated on what’s happening at the practice, community events and initiatives and important health information.

What’s new?

This September, Horizon Healthcare hosted two incredibly successful Flu Fun Days for children’s flu vaccinations, and we are thrilled to announce that our efforts helped increase flu vaccine uptake by more than half! These fun-filled days brought together families and children to receive their flu shots in a relaxed, engaging atmosphere, ensuring more of our young ones are protected this flu season.

We are also excited to share that thanks to the generosity of those who attended, we raised a fantastic £281.01 for Family Support Derbyshire, a local charity providing support to families in Derby City, Amber Valley and South Derbyshire.

NHS Diabetes Prevention Programme

While type 1 diabetes cannot be prevented, type 2 diabetes is largely avoidable through healthy lifestyle changes. With around nine out of ten people with diabetes having type 2, there are currently two million people in England at high risk of developing this condition. Type 2 diabetes is a leading cause of preventable sight loss among working-age adults and a major contributor to serious health issues like kidney failure, heart attack, and stroke. It also doubles the risk of dying in hospital for people infected with COVID-19, compared to those without the condition.

Along with the serious health impacts, type 2 diabetes treatment accounts for around 10% of the NHS budget. But there’s good news—there are steps you can take to reduce your risk.The Healthier You NHS Diabetes Prevention Programme

The Healthier You NHS Diabetes Prevention Programme identifies individuals at risk of developing type 2 diabetes and supports them through a nine-month, evidence-based lifestyle change programme.

The programme is available in two formats:

Face-to-face group service: You’ll receive personalised support to manage your weight, improve your diet, and become more physically active—all proven to lower the risk of developing type 2 diabetes.

Digital service: Similar to the group option, but using digital tools like wearable fitness trackers, apps with health coaches, online peer support groups, and goal-setting platforms.Research shows that people who complete the Healthier You programme cut their risk of developing type 2 diabetes by more than a third, preventing thousands of new cases each year.

Know Your Risk of Type 2 Diabetes

You can significantly reduce your chances of developing type 2 diabetes by making positive changes to your diet, physical activity, and weight management. To find out if you’re at risk, you can:

• Use the ‘Know Your Risk’ tool at riskscore.diabetes.org.uk

• Take up a free NHS Health Check (available for those aged 40-74)

• Ask your GP practice to check if you’re at risk

Addressing missed appointments

We need your help to improve our services and ensure timely care for all patients. In August, 336 appointments were missed. That’s over 87 hours of clinical time wasted.

When appointments are missed, it not only affects the individual patients but also limits our ability to offer those time slots to others in need of care. 

You can help by cancelling any appointments you can no longer attend. Either via phone or on our website

Join our patient participation group (PPG)

We believe in working together with our patients to improve our services. Our Patient Participation Group (PPG) is a vital part of this effort, providing a platform for patients to share their insights and feedback.

The PPG is a group of patients who meet regularly to discuss and influence the way services are provided at Horizon. Members have the opportunity to suggest improvements, share experiences and help to shape the future of our practice.
