We only invite patients to complete a New Patient Health Check if it is clinically appropriate.
This is usually because:
- You have not had recent checks (e.g. blood tests or observations) in the last 12 months, and
- You are not already being monitored through a long-term condition review
Why This Is Important
This helps us to:
- Ensure your medical record is up to date
- Identify any health needs early
- Avoid unnecessary or duplicate tests
