If you have a long-term condition (such as asthma, COPD, diabetes, high blood pressure or heart disease etc), you need a yearly review.
This helps us make sure your treatment is safe and working well.
What you need to do
Step 1 โ Book a nurse/HCA appointment
All your checks will be done in one appointment with a practice nurse/HCA.

๐ข Enter PIN given in message for nurse appt. followed by the * key
At this appointment, the nurse/HCA may:
- Take blood samples
- Ask for a urine sample (if needed)
- Check your blood pressure
- Check your weight/height
- Carry out other checks for your condition
If your blood test is done at the surgery, we will print the form for you.
If you need to go elsewhere for your blood test, you can collect the form from reception or request it on our website.
โ Please bring a urine sample if you have been asked to provide one.
Step 2 โ Book your telephone medication review
After your nurse appointment, you must book a telephone medication review.
Please book this at least 1 week after your nurse appointment. This allows time for your blood test results to come back.

During this call, your clinician will:
- Go through your results
- Discuss your condition
- Make changes to your medication if needed
- Agree your treatment plan with you

Completing the questionnaire is helpful but not essential.
