Most medical reports, letters and forms requested for third parties are not part of NHS contracted services and may be chargeable.
If a fee applies, we will contact you before any work is undertaken.
Most requests are completed within one calendar month. Complex requests, including Subject Access Requests, may take longer where permitted by legislation.
Need to request or chase a letter or report from us?
Use our automated telephone service, available 24 hours a day. Call 01332 502735, select option 1, then option 2, followed by option 4, and follow the prompts. Your query will be sent directly to the communications team. You will receive a response within 3 working days, although any work can take up to 30 working days to process.
Alternatively you can complete the below form – requests via this method can take up to 5 working days for a response but again any work can take up to 30 working days to process.
