You will be asked to confirm:
- your date of birth
- your contact number
Please ensure your contact details are up to date.
Once verified, choose:
- Option 1
- then Option 4
The system will check for available appointments and guide you through the booking process.

You can use this service if you:
- are registered at Horizon Healthcare
- have been invited for a medication review
Important
Medication reviews help us ensure your medication remains safe and appropriate.
Some reviews may require:
- blood pressure readings
- blood tests
- monitoring checks
- review by a pharmacist or clinician
If your monitoring is overdue, your medication request may take longer to process while a clinician reviews whether it is safe to continue prescribing.
