Why You Have Been Asked to Complete This

We only invite patients to complete a New Patient Health Check if it is clinically appropriate.

This is usually because:

  • You have not had recent checks (e.g. blood tests or observations) in the last 12 months, and
  • You are not already being monitored through a long-term condition review

Why This Is Important

This helps us to:

  • Ensure your medical record is up to date
  • Identify any health needs early
  • Avoid unnecessary or duplicate tests

Important Information

  • This is a non-urgent request
  • Please allow up to 4 working days for a response

If you need urgent medical advice, please contact the surgery or use NHS 111.

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
