Request an appointment with a nurse or healthcare assistant

To help manage appointments safely and efficiently, some Nurse and Healthcare Assistant (HCA) appointments must now be requested using the form below.

Once we have reviewed your request and confirmed eligibility, you may be sent instructions – including a PIN number – to book your own appointment using our automated telephone booking system.

We no longer allow patients to directly access nurse appointment booking via the main phone menu. This is to ensure appointments are only booked for appropriate needs. You must wait for your request to be reviewed.

We usually respond to requests within 4 working days during opening times.

Contraception

Many contraception methods can be supplied directly by SH:24 and participating pharmacies.

Blood pressure checks

Free blood pressure checks are available at many community pharmacies for patients aged over 40 without diagnosed hypertension.

Severe difficulty breathing 
not being able to get words out, choking or gasping

Heavy bleeding 
spraying, pouring or enough to make a puddle

Severe injuries 
after a serious accident

Seizure (fit) 
shaking or jerking because of a fit, or unconscious (can’t be woken up)

Sudden, rapid swelling 
of the lips, mouth, throat or tongue

Labour or childbirth 
waters breaking, more frequent intense cramps (contractions), baby coming, or just born

British Sign Language (BSL) speakers can make a video call to 999.

Deaf people can use 18000 to contact 999 using text relay.

  • Press 3 to move to the next available date
  • Press 1 to move to the next available appointment time
  • Press 0 to book the offered appointment
