Use this service to update your personal details, including:

  • Address
  • Name
  • Telephone number
  • Email address

It is important that we have up-to-date contact details so that we can contact you when necessary regarding your healthcare.

You Can Use This Service If You:

  • Are registered with Horizon Healthcare

Important Information

Change of Address

Please ensure you include:

  • House number or name
  • Flat or room number (where applicable)
  • Building name (where applicable)
  • Full postcode

Patients aged 18 and over must submit their own change of address request.

Parents and guardians may submit address changes for children under the age of 18.

Change of Name

If your name has changed due to marriage, civil partnership, divorce or deed poll, you will be required to upload supporting documentation before we can update your medical record.

Examples include:

  • Marriage certificate
  • Civil partnership certificate
  • Decree absolute
  • Deed poll certificate

Contact Details

Please ensure your telephone number and email address are correct so that we can contact you regarding appointments, results and other important healthcare information.

To protect the accuracy of patient records, we may contact you if we require further information before making any changes.

Before You Start

We will ask you for:

  • Your name
  • Date of birth
  • Contact details
  • Details of the changes required
  • Supporting documents (for name changes)

This form should only be completed if your telephone number, email address or home address has changed.

If your contact details have NOT changed, please do NOT complete this form.

If you have received a message from the NHS asking you to confirm your GP registration, please respond directly to the NHS as instructed. You do NOT need to complete this form simply to remain registered with Horizon Healthcare.

We are currently receiving a large number of forms where no changes are required. Each submission must be checked by a member of staff, which may delay the processing of genuine requests to update patient records.
