Medication review telephone appointment

We have an automated telephone system which is available 24/7 365 days of the year.

Use can use this service to book your own telephone appointment for your annual medication review.

If in your original letter, you were asked for tests i.e. bloods, urine and blood pressure, we can only do your review if these have been done and the results have been received.

If you book your medication review before the results are back, you will be asked to rebook – a full review cannot be done without these results.

You can use this service if you:

  • have long term conditions and are due your annual review
  • have been invited to do so

To use the automated telephone service, please call 01332 502735

You will be asked for your date of birth and contact number to verify (please ensure your contact details are up to date).

Once verified – please choose option 1, then option 4. The system will then look for available appointments for you to book.

We kindly ask that you book and manage your own appointments where possible, as this helps free up staff time and ensures the system runs more efficiently.

If you are unable to use the telephone system, ie you have hearing difficulties or your contact details are not up to date then use the form below – please note that these can take up to 7 working days to process.
