If you have been invited for an annual medication review, you can usually book your own telephone appointment using our automated telephone service.
Before You Book
If your invitation asked you to complete any tests (for example blood tests, urine tests or blood pressure readings), these must be completed and the results received before your medication review can take place.
If you book a medication review before your results are available, you will be asked to rebook your appointment.
Book Using Our Automated Telephone Service

To verify your identity, you will be asked for:
- Your date of birth
- Your contact telephone number
Please ensure your contact details are up to date before calling.
Once verified, select:
Option 1 → Option 4
The system will then offer any available medication review appointments.
Why Use the Automated Service?
Booking your own appointment is often quicker and allows you to choose from available appointments without waiting for the practice to process a request.
Using the automated service also helps free up staff time for patients who require additional support.
Unable to Use the Automated Telephone Service?
You may use the online form below if:
- You have hearing or communication difficulties
- Your contact details are incorrect and you are unable to verify your identity
- You have another reason why you are unable to use the automated service
