- to request medication that is not on your repeat prescription
- for medication queries or pharmacy issues
Before you start
We will ask you for:
- your name, date of birth, postcode, telephone number, and email address
- details of the person you are completing the form for (if applicable)
- the medication you would like to request
Important
Some medication requests may take longer if:
- your medication review is overdue
- monitoring blood tests or blood pressure readings are required
- a clinician needs to review the request before prescribing safely
