Before you start
We will ask you for:
- your name, date of birth, postcode, telephone number, and email address
- details of the person you are completing the form for (if applicable)
- the medication you would like to request
Please note
- Requests may take up to 4 working days to be processed by the surgery.
- Pharmacies may require additional time to prepare your medication.
- Requests submitted significantly earlier than expected may require review before they can be issued.
- Medications that require monitoring may be reduced, changed or stopped if required reviews, blood tests or other safety checks have not been completed.
