To book a routine appointment with a Mental Health Practitioner using our new automated service, available 24 hours a day, 365 days a year, please call 01332 502735. Choose Option 1, followed by option 5 and then follow the prompts provided. You will be able to book any available mental health appointments directly.

Alternatively you can request an appointment via the form below – please note that these can take up to 7 working days to process.

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

Call 999 or go to A&E now if:

  • someone’s life is at risk – for example they have seriously injured themselves or taken an overdose
  • you do not feel you can keep yourself or someone else safe

A mental health emergency should be taken as seriously as a physical one. You will not be wasting anyone’s time.
