Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
Please note:
- Test results are reviewed by an appropriate clinician before being filed in your medical record.
- If your clinician needs to discuss the result with you or arrange further treatment, tests or an appointment, we will normally contact you.
- If you have not been contacted, this usually means that no urgent action is required.
- Many test results can be viewed in the NHS App once they have been reviewed and filed.
- Some test results can take several days or weeks to be received and reviewed.
Tests requested by other services
Horizon Healthcare can usually only view and action results for tests that have been requested by the practice.
If your test was requested by:
- A hospital department
- Accident & Emergency (A&E)
- An outpatient clinic
- A community service
- Another healthcare provider
The result will normally be returned directly to the clinician or service that requested the test.
If you are waiting for the result of a test requested by another service, please contact that service directly for an update.
If your symptoms have worsened
If your condition has worsened whilst waiting for a result, please do not wait for the result.
Please submit a Get Help request so that your symptoms can be assessed by an appropriate clinician.
