Before You Start β Please Read
- This form does not guarantee an appointment. Your request will be reviewed by our Care Navigation Team. If an appointment is appropriate, it will be booked based on clinical need.
- Appointments will be face-to-face only. We do not offer routine telephone, video or message consultations. If your condition requires clinical assessment, this will be in person.
- Allow up to 3 working days for a response. Your form will be reviewed the same working day (unless submitted after closing time) but you may not receive a response immediately.
- Use the NHS App to avoid missed messages. Due to NHS Digital changes, if you have the app installed, messages will go there first. If not read within 2 hours, they will default to SMS. We cannot override this system β if you do not have notifications enabled, you may miss appointment offers.
- Check self-help options and your local pharmacy first. Many common ailments are best treated by a pharmacy team. Where appropriate, we will refer you to Pharmacy First instead of booking a GP/Acute appointment.
- Depending on the nature of your symptoms and appointment availability, you may be offered a face-to-face appointment at the surgery or signposted to a more appropriate service such as your community pharmacy or the local walk-in centre.

Signs of a heart attack
chest pain, pressure, heaviness, tightness or squeezing across the chest
Signs of a stroke
face dropping on one side, canβt hold both arms up, difficulty speaking
Sudden confusion (delirium)
cannot be sure of own name or age
Suicide attempt
by taking something or self-harming
Severe difficulty breathing
not being able to get words out, choking or gasping
Heavy bleeding
spraying, pouring or enough to make a puddle
Severe injuries
after a serious accident
Seizure (fit)
shaking or jerking because of a fit, or unconscious (canβt be woken up)
Sudden, rapid swelling
of the lips, mouth, throat or tongue
Labour or childbirth
waters breaking, more frequent intense cramps (contractions), baby coming, or just born
British Sign Language (BSL) speakers can make a video call to 999.
Deaf people can use 18000 to contact 999 using text relay.
