To help manage our appointments safely and efficiently, we have introduced a new system for booking some Nurse and Healthcare Assistant (HCA) appointments.
Please complete the form below to let us know which type of appointment you are requesting. Once we have reviewed your request and confirmed your eligibility, we may be sent instructions – including a PIN number – to book your own appointment using our automated telephone booking system.
π Important:
We no longer allow patients to directly access nurse appointment booking via the main phone menu. This is to ensure appointments are only booked for appropriate needs. You must wait for your request to be reviewed.
We will respond to requests within 4 working hours during opening times.

Before requesting a nurse or HCA appointment, please check whether your needs could be met more quickly through another service:

- if applicable, the details of the person you are completing the form on behalf of

Signs of a heart attack
chest pain, pressure, heaviness, tightness or squeezing across the chest
Signs of a stroke
face dropping on one side, canβt hold both arms up, difficulty speaking
Sudden confusion (delirium)
cannot be sure of own name or age
Suicide attempt
by taking something or self-harming
Severe difficulty breathing
not being able to get words out, choking or gasping
Heavy bleeding
spraying, pouring or enough to make a puddle
Severe injuries
after a serious accident
Seizure (fit)
shaking or jerking because of a fit, or unconscious (canβt be woken up)
Sudden, rapid swelling
of the lips, mouth, throat or tongue
Labour or childbirth
waters breaking, more frequent intense cramps (contractions), baby coming, or just born
British Sign Language (BSL) speakers can make a video call to 999.
Deaf people can use 18000 to contact 999 using text relay.
