Online Requests
You can also request a fit note using our online form.
Most requests are processed within 5 working days.
If your condition has worsened or you require medical advice, please use our Get Help form instead.
Before You Start
We will ask you:
- Whether this is a new fit note or an extension to an existing fit note
- How long you have been off work
- Details of your illness or condition
- Whether you have already been assessed for the condition
- Whether you are currently under the care of a hospital specialist
If your condition has not previously been assessed, you may need an appointment before a fit note can be issued.
