Medication review telephone appointment

We have a new automated telephone system which is available 24/7 365 days of the year.

Use can use this service to request a telephone appointment for your annual medication review.

If in your original letter, you were asked for tests i.e. bloods, urine and blood pressure, we can only do your review if these have been done.

You can use this service if you:

  • are registered at the surgery
  • have long term conditions and are due your annual review
  • have been invited to do so

To use the automated telephone service, please call 01332 502735

You will be asked for your date of birth and contact number to verify (please ensure your contact details are up to date).

Once verified – please choose option 1, then option 4. The system will then look for available appointments for you to book.

Alternatively you can complete a form to request an appointment – this can take up to 5 working days to process.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
